Enrollment Instructions

Thank you for your interest in MSH OnsiteHealth for Hospital Professionals.

In order to gain access to MSH OnsiteHealth you must complete two simple steps outlined below. After you have completed these steps your request will be sent to the Information Technology Department for processing. Once the request has been processed you will receive an e-mail regarding your request status and information for logging on for the first time.
  1. Read the System Requirements on the following page to ensure you are aware of the appropriate hardware needed to support MSH OnsiteHealth. Select "Continue".
  2. Fill out enrollment form. Select "Submit this form". You will receive a confirmation number after submission. Please keep this number for future reference.

The Information Technology Department will process your enrollment request. You will receive an email with your request status shortly after submission. If you are approved for access you will receive a user name and indication of your password, as well as detailed instructions to assist you in logging into MSH OnsiteHealth.
